About the Role:

Gentiva Hospice is seeking a skilled Social Worker (MSW) to join our interdisciplinary team. As a Social Worker (MSW), you will provide psychosocial support to patients and their families, utilizing your expertise to identify needs, goals, and interventions that align with our established plan of care.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments to identify patient and family needs
  • Develop and implement individualized plans of care that address psychosocial needs
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
  • Provide emotional support and counseling to patients and their families

Requirements:

  • Master's Degree in Social Work from a CSWE-accredited school
  • Licensure as required by the state in which the hospice is located
  • One year of experience as a Social Worker in a healthcare and/or hospice setting, with three to five years preferred
