- Education and Support: Educate patients and their families on proper home health care procedures, disease management, and healthy lifestyle habits to promote independence and self-care.
- Collaboration and Teamwork: Work collaboratively with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
- Education: Associate degree in Nursing from an accredited institution; Bachelor's degree in Nursing preferred.
- Licensure: Current and valid Connecticut Licensed Practical Nurse license.
- Experience: One year of LPN experience in home health care or a related field.
- Certifications: Current CPR certification.
- Skills: Strong interpersonal and communication skills, with the ability to build trust and rapport with patients and families. Computer literacy and strong organizational skills are also essential.
The physical demands of this position include standing, walking, sitting, and reaching. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required include close vision, distance vision, and the ability to adjust focus. The work environment is typically low to moderate noise level.
