Role Responsibilities
The Physical Therapist (PT, DPT, RPT) in Home Health is tasked with assessing and evaluating patient care needs concerning mobility, endurance, pain, and other health conditions. Based on these evaluations, you will develop treatment plans, implement interventions to enhance patient well-being, and monitor progress.
- Assess the patient's functional status and therapy requirements, collaborating with physicians to create a comprehensive therapy plan.
- Monitor and report on the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
- Administer treatments aimed at alleviating pain, restoring function, and maintaining optimal physical performance according to established standards.
- Educate and inform healthcare team members, patients, and their families about the care plan, functional limitations, and progress towards therapeutic goals.
Licensure Requirements
- Valid Physical Therapy license in the state of practice.
- Current CPR certification.
- Possession of a valid driver's license, vehicle insurance, and access to reliable transportation.
