About the Role

Hire Velocity is seeking a skilled Medical Social Worker to join our team in the Greater Grand Rapids area. As a Hospice Social Worker, you will play a vital role in delivering high-quality social work services to patients and their families.

Key Responsibilities
  • Conduct initial emotional, spiritual, and psychosocial assessments to identify patient needs and develop personalized care plans.
  • Provide ongoing counseling, bereavement services, and community education to support patients and their families.
  • Collaborate with the Hospice Interdisciplinary Group (IDG) to ensure comprehensive care and coordination.
  • Develop and maintain relationships with community resources and healthcare providers to ensure seamless care transitions.
Requirements
  • Full Licensed Bachelor's Social Worker (LBSW) or Full Licensed Master's Social Worker (LMSW) in Michigan.
  • Minimum 2 years of experience as a social worker, with a preference for experience in hospice care.
  • Current valid driver's license, auto liability insurance, and reliable transportation.
  • Active patient contact within the past three years, preferred.
Preferred Qualifications
  • MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE) and 1 year of experience in a healthcare setting.
  • Baccalaureate degree in social work (BSW) from a school of social work accredited by the CSWE, and 1 year of experience in a healthcare setting, with supervision by a MSW from a school of social work accredited by the CSWE and who has 1 year of experience in a healthcare setting.
  • Baccalaureate degree in psychology, sociology, or other field related to social work, and at least 1 year of social work experience in a healthcare setting, with supervision by a MSW from a school of social work accredited by the CSWE and who has 1 year of experience in a healthcare setting.
