Job Summary

The Primary Care Licensed Practical Nurse (LPN) plays a vital role in delivering quality nursing care in a community setting. Working closely with healthcare providers, this role involves a range of clinical responsibilities that demand a combination of basic and advanced nursing expertise.

Key Responsibilities
  • Provides direct patient care support in a variety of settings, utilizing primary and palliative care principles to promote patient well-being and empowerment.
  • Coordinates, schedules, and conducts annual wellness visits with patients, families, and caregivers, ensuring comprehensive care and optimal scheduling of patient visits.
  • Performs medication reconciliation through patient interview, review of medical records, and consulting with other healthcare providers involved in the patient's care, addressing any care gaps and recommended preventative measures.
  • Engages patients and families in advance care planning, discussing goals of care and answering general questions about advanced directives, and assisting patients with documentation if requested.
  • Educates patients about their health conditions, treatment options, and self-management strategies to promote active participation in their care.
  • Coordinates, schedules, and accurately performs point of care testing, communicating findings to the healthcare team for further evaluation and treatment.
  • Communicates test results with patients when deemed appropriate by the provider or payer.
  • Assesses vaccination needs, discusses vaccination options, obtains informed consent, prepares and administers vaccines, monitors for adverse reactions, and provides post-vaccine education and documentation.
  • Recognizes signs and symptoms of common and routine medical conditions/illnesses, abnormal versus new/existing, and collaborates with patient's provider when identified.
  • Works closely with members of the primary care interdisciplinary team or payor staff to ensure coordinated and comprehensive care.
Requirements
  • Graduate of an accredited LPN training program.
  • Current unencumbered LPN license in the State of Oregon.
  • Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
  • CPR certification at hire or within 6 months in position.
Preferred Qualifications
  • Minimum 2 years' experience in primary care, geriatrics, home health, or hospice.
Working Conditions

This role requires travel outside of the workplace at least weekly, and the employee's personal vehicle may be used. Driving infractions will be monitored in accordance with organizational policy.
