Role Summary:
As an Ambulatory Clinical Pharmacist, your responsibilities will include:
  • Implementing pharmacy services and clinical programs in accordance with established policies and regulations.
  • Reviewing patient records to assess medication therapy appropriateness and identifying candidates for outreach programs.
  • Evaluating patient conditions and addressing barriers to medication adherence to ensure comprehensive care.
  • Identifying unaddressed health issues and referring patients to the appropriate healthcare providers.
  • Formulating medication plans that minimize adverse effects and enhance therapeutic outcomes.
  • Providing healthcare professionals with essential medication information to support safe prescribing practices.
  • Collaborating with the healthcare team to optimize patient care and outcomes.
  • Ensuring proper procurement, storage, and administration of medications within the clinical environment.
  • Prescribing and modifying medications, adjusting dosages, and ordering laboratory tests as per collaborative practice agreements.
  • Maintaining accurate documentation of medication plans and patient progress to facilitate efficient care and billing.
  • Participating in quality improvement initiatives through data collection and analysis to enhance service delivery.

Qualifications:
  • PharmD Degree
  • A minimum of two (2) years of experience in a retail or outpatient pharmacy setting, or completion of a pharmacy residency program.
Preferred Qualifications:
  • Completion of a PGY2 Ambulatory Pharmacy Residency or equivalent experience.
  • Board certification in a relevant area (e.g., BCACP, BCPS).
Physical Requirements:
Ability to perform tasks such as hearing, lifting, manual dexterity, and mobility.

Work Schedule:
This position typically operates on a Monday to Friday schedule during daytime hours, with no weekend or holiday shifts.

Compensation:
The hourly wage for this role is competitive and commensurate with experience.
