Client Information:
The client is located in a residential area and requires assistance from Monday to Wednesday, 9:00 am to 1:00 pm.
Client Background: The client lives independently, is a widow, and has no pets.
Health Conditions: The client has been diagnosed with Osteoarthritis, Osteoporosis, a recovering Cracked Pelvis, Aphasia, Hypothyroidism, GE Reflux, and is at risk of falls. Allergies: Medication - Pyridium, Allopurinol, Bactrim; Food - None

Assistance Required: The client needs help with preparing breakfast, cleaning the kitchen, doing weekly laundry, and changing bed linens. Support includes assistance with showering, light housekeeping, monitoring transfers, ensuring hydration, running errands, and providing medication reminders. Experience in Dementia care is crucial.

Activities of Daily Living (ADLs):
Mobility: The client uses a rollator indoors and can walk short distances without it. Assistance is necessary for sitting and standing, with additional care required due to fall risk.
Bathing/Dressing/Grooming: Support is needed for dressing, particularly for tops due to shoulder discomfort. Assistance with showering, drying off, and hair care (wig) is required. The client can brush teeth and select clothes independently.
Eating: Help is needed with meal preparation as the client struggles to stand for extended periods. Support includes serving breakfast and lunch, and grocery assistance may be necessary.
Communication: The client communicates effectively, is articulate, and responsive. Memory lapses necessitate reminders.

Qualifications:
Minimum of 1 year of experience as an in-home Caregiver, CNA, or in a related field
Strong written and verbal communication skills
Valid Driver's License and appropriate auto insurance
Consent to various background checks and drug/alcohol testing
Completion of TB test
