Job Summary

We are seeking a skilled Physical Therapist to join our team at Crystal Clinic. As a PRN Physical Therapist, you will be responsible for evaluating, treating, and creating an appropriate plan of care for post-surgical orthopedic patients in our acute care setting.

Key Responsibilities
  • Evaluate and re-evaluate patient's functional ability and physical status consistent with patient diagnosis/surgical procedure and physician referral
  • Develop a plan of care and measurable goals consistent with evaluation findings and physician referral
  • Administer various physical therapy treatments to patients based on physician orders
  • Assist in keeping patient-related records and preparing reports
  • Consult with physicians, nursing team, case management, and other interdisciplinary team members
  • Provide patient and family education
  • Maintain records and process appropriate charges
Requirements
  • Doctorate or Master's Degree in Physical Therapy or Bachelor's Degree and certificate in Physical Therapy
  • Minimum Three (3) to Five (5) years of demonstrated experience in the field or profession preferred
  • Current License issued by the Occupational Therapy/Physical Therapy/Athletic Trainers Board to practice Physical Therapy in the State of Ohio
  • BLS required
  • Minimum 24 hours per month
