We are seeking a highly skilled and compassionate Registered Nurse to join our Hospice team as an Admission RN. As a key member of our interdisciplinary team, you will provide exceptional care to patients and their families, ensuring a seamless transition to our hospice services.
Responsibilities- Conduct comprehensive assessments of patients' physical, functional, psychosocial, and environmental needs
- Develop and implement individualized plans of care to ensure quality and continuity of care
- Collaborate with patients, families, and healthcare professionals to coordinate care and address complex needs
- Provide education and support to patients and families on disease management, self-care, and end-of-life care
- Monitor and evaluate patient outcomes, making adjustments to care plans as needed
- Current RN licensure and 2+ years of experience in a clinical care setting, preferably in hospice care
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment
- Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities
- Ability to work flexible hours, including evenings and weekends, and to travel locally as needed
