About the RoleWe are seeking a highly skilled and compassionate MA/LPN/RN to join our care team as a Home Health Assessment Professional. As a key member of our team, you will play a vital role in delivering high-quality care to our patients, leveraging your exceptional communication skills and technical expertise to drive positive health behavior change and improved patient outcomes.
Key Responsibilities- Conduct routine primary care nursing tasks, including vital signs, point-of-care tests, and maintaining accurate patient medical records.
- Utilize motivational communication techniques and strategies to educate patients on managing their acute or chronic medical conditions, promoting health, wellness, and preventing emergency room utilization and hospitalization.
- Significantly impact longitudinal patient engagement in our RPM program(s) by providing preventive health and disease management education and coaching.
- Collaborate with our care team to develop and implement processes and best practices that enhance patient care and outcomes.
- Bilingual Russian speaking
- Experience working with Medicare patients
- Interest in professional leadership growth and development opportunities with a growing organization
- Technically savvy and comfortable using tools such as iPads for charting
- Valid driver's license with a clean driving record
- Ability to lift and carry equipment up to 20 pounds
- Comfortable driving to patient's homes in NYC and providing care
- Comfortable seeing patients independently and delivering a positive experience
- Full COVID-19 vaccination is a requirement for this position. Medical and religious exemptions may be granted with proper documentation.
