Job Summary

The State of Delaware is seeking a skilled Active Treatment Facilitator to join our team. As a key member of our department, you will provide active treatment and assistance with activities of daily living to intermediate care residents.

Responsibilities include:

  • Providing active interaction and assistance with feeding, toileting, bathing, socialization, safety, personal hygiene, and ambulation
  • Conducting periodic rounds to ensure the safety and well-being of clients
  • Supporting residents in fulfilling treatment and learning plans developed by professional staff
  • Implementing treatment plans through individual and group therapeutic sessions
  • Observing and recording any unusual behavior or significant behavior changes

Requirements:

  • Knowledge of instructing and assisting clients in developing functional, social, recreational, and vocational skills
  • Knowledge of working with persons with disabilities
  • Knowledge of applying laws, rules, regulations, standards, policies, and procedures
  • Knowledge of narrative report writing
