Job Title: Case Manager

Community Housing Innovations (CHI) is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide supportive case management services to tenants of our permanent affordable supportive housing program.

Job Summary:

The Case Manager will work with tenants to build and enhance independent living skills, including timely rent payments and healthy housekeeping, to promote housing stability. This is a full-time position with a competitive salary and benefits package.

Responsibilities:
  • Screen DHS referrals for eligibility and certification for program enrollment
  • Conduct tenant intake and needs assessment to determine independent living strengths and weaknesses
  • Develop independent living plans (ILPs) to ensure housing stability
  • Provide case management to tenants in accordance with ILPs to track progress and milestone achievements
  • Coordinate workshops or provide trainings that promote independent living and housing stability
  • Engage tenants in supportive and service programs such as Workforce Development and Financial Literacy
  • Refer tenants to community-based services to meet ILP goals
  • Advocate on behalf of tenants for entitlements and benefits, employment opportunities, and community-based services
  • Link tenants to employment support services such as resume building, interviewing skills development, and job skills development
  • Ensure proper and timely follow-up for incidents reporting to DHS and CHI
  • Submit program reports correctly and on schedule to CHI, DHS, and oversight agencies
Requirements:
  • Bachelor's Degree in Social Work or Counseling preferred
  • 3+ years of experience working in supportive housing or a similar field
  • Professional verbal and written communication skills
  • Professional competency in MS Word, Excel, Outlook, and the internet
  • Ability to develop and manage program budgets
  • Ability to make unit visits (walk distances, climb stairs, etc.)
  • Ability to prioritize, make appropriate decisions, and judgment calls
