We are seeking a skilled Speech Therapist to join our team at Genesis Home Care. As a Speech Therapist, you will provide home-based skilled speech therapy care for patients as directed by the attending physician.
Key Responsibilities- Develop an individualized ST plan of care and make adjustments as needs change.
- Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning.
- Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
- Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.
- Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
- Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy.
- Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Speech Therapist with a current license in the state of employment.
- Minimum one-year experience as a ST in an acute care setting. Home care experience preferred.
- Valid driver's license and auto insurance in your name as a driver.
- Capable of all physical demands.
