Job Summary

Catholic Health Initiatives is seeking a skilled Registered Nurse to join our team as a Patient Care Coordinator RN. This role is responsible for coordinating daily patient care activities, ensuring effective communication between nursing, medical staff, and ancillary departments, and maintaining patient-specific nursing care plans.

Key Responsibilities
  • Assist the Director of Nursing in establishing goals and objectives that enhance the competencies of the department and actively support LakeWoods strategic goals.
  • Responsible for Daily Nursing Assignments, assessing, identifying, and communicating unit staffing needs for current and oncoming shifts, and assigning admissions and/or transfers based on patient acuity level, nurse/patient ratio, and nursing skill levels.
  • Coordinate daily patient care activities with acute care nursing staff and other related services.
  • Provide, maintain, and coordinate effective communications between nursing, medical staff, and ancillary departments.
  • Ensure patient-specific nursing care plans are developed, updated, and evaluated for effectiveness.
  • Initiate problem-solving processes with nursing staff as patient care concerns arise on a daily basis.
  • Communicate with staff to assure assignment made is appropriate to promote team building and cohesiveness.
  • Knowledgeable and competent to care for patients in the emergency room and outpatient setting.
  • Retain overall accountability for the workflow for their shift, and remains accountable if duties are delegated to another qualified staff member.
  • Patient care areas include medical, surgical, pediatric, geriatric, emergency, same day surgery, swing bed, outpatients, respite.
Requirements
  • Graduate of an accredited school of nursing.
  • Current licensure as Registered Nurse by the Minnesota State Board of Nursing.
  • BLS.
  • ACLS, PALS TNCC obtained within 12 months of hire will be acquired and maintained.
Compensation

$46.29/hour.
