Job Summary

We are seeking a highly skilled Clinical Case Manager to join our team at UP Health System - Marquette. As a Clinical Case Manager, you will play a critical role in promoting continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.

Key Responsibilities
  • Consult with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of medical care, including admission, length of stay, transfer, and discharge.
  • Monitor patient and family satisfaction, responding to questions and complaints regarding care.
  • Participate in discharge planning, coordinating patient transfers to other facilities and community resources, and providing discharge education and resource referrals.
  • Perform chart reviews to identify actual or potential issues with service delivery, patient outcomes, satisfaction, compliance, cost, and reimbursement.
Requirements
  • Minimum Education: Associates degree in Nursing; Bachelor's degree in Nursing (Preferred)
  • Required Skills: Critical thinking skills, decisive judgment, and ability to work with minimal supervision
  • Licenses: Registered Nurse (RN) in State of Michigan; Certifications: Basic Life Support Health Care Provider (BLS-HCP)
  • Minimum Work Experience: 5 years nursing experience; 5 years hospital experience (Preferred)
