Nurse Care Coordinator Role at Mass General Brigham

We are seeking a skilled Registered Nurse to join our Case Management team as a Nurse Care Coordinator.
About the JobThis is a full-time opportunity to work in a dynamic environment where you will be responsible for coordinating patient care services along the continuum of care. As a key member of our healthcare team, you will evaluate, predict, and facilitate the trajectory of patient care.
Responsibilities:- Evaluate patients' health needs and develop individualized care plans to ensure efficient and effective delivery of care.
- Collaborate with interdisciplinary teams to coordinate care services, including referrals to community resources and specialists.
- Analyze data to identify trends and areas for improvement in patient care and outcomes.
- Develop and implement strategies to improve patient engagement and satisfaction.
To be successful in this role, you will need:
- A current RN license in the state of Massachusetts.
- Minimum 2 years of Med/Surg experience required.
- Case Manager certification preferred.
- ADN required, but BSN strongly preferred.
- Acute hospital experience preferred.
