RN Case Manager

We are seeking a skilled RN Case Manager to join our team at Loyola Medicine. As a key member of our multidisciplinary team, you will work closely with physicians, staff, and payers to ensure patient progress along the continuum of care in an efficient and cost-effective manner.
Key Responsibilities:
- Conduct patient assessments to identify complex clinical discharge needs and develop individualized plans of care.
- Collaborate with the multidisciplinary team to ensure patient goals are set and negotiated with patient/family, and educate patient/family regarding available options of care throughout the continuum.
- Facilitate patient education for safe and effective discharge, and conduct daily multidisciplinary rounds to identify and address care coordination barriers.
Requirements:
- Minimum Education: Required: Bachelors Degree OR equivalent training acquired via work experience or education. Preferred: Masters Degree
- Minimum Experience: Required: 3-5 years of previous job-related experience Preferred: 6-10 years of previous job-related experience
- Recent clinical experience in medicine, surgery, critical care, telemetry, or ED is required. Recent Case management experience is preferred.
