Hospice Registered Nurse Job Description

About the Role:

We are seeking a skilled and compassionate Hospice Registered Nurse to join our interdisciplinary team at Healthkeeperz, Inc. As a key member of our team, you will play a vital role in providing high-quality care to our clients and their families.

Key Responsibilities:

  • Assess clients' physical, emotional, and psychosocial needs and develop individualized care plans.
  • Coordinate care with the interdisciplinary team, including social workers, chaplains, and volunteers.
  • Provide skilled nursing care, education, and support to clients and their families.
  • Participate in bereavement support and provide emotional support to clients and families.
  • Document and ensure accurate use of medications, DME, and supplies.
  • Participate in weekly meetings and IDG meetings to ensure seamless care coordination.

Requirements:

  • Active NC nursing license.
  • At least one year of hospice or home health nursing experience preferred.
  • Ability to speak in front of groups and maintain high-quality customer service approach.
  • Working knowledge of computer programs desired.

Physical Demands:

The ability to sit for long periods, frequent bending, reaching, lifting, talking, and listening are required for this role.
