Job Summary

St. David's Healthcare at Home is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As a Home Health LVN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Administer skilled nursing care to patients in their homes, adhering to physician orders and the Plan of Care.
  • Conduct ongoing assessments to identify physical, psychosocial, and environmental needs of patients, documenting findings and communicating with the supervising RN and Director of Professional Services.
  • Provide basic and specialized teaching to patients and caregivers, as ordered in the Plan of Care, to promote health and wellness.
  • Collaborate with other healthcare professionals to ensure comprehensive care and address any concerns or issues that may arise.
Requirements
  • Graduate of an accredited LPN/LVN program.
  • Minimum of one year experience as an LPN/LVN in a clinical care setting.
  • Home care experience preferred.
  • Must be licensed as an LPN/LVN in good standing in the state(s) where services will be provided.
  • Current BCLS certification prior to delivering patient care.
  • Reliable transportation and proof of valid automobile liability insurance.
