Occupational Therapist

BAYADA Home Health Care is seeking a skilled Occupational Therapist to join our team in providing high-quality care to clients in their homes.

As a Home Health Occupational Therapist, you will work closely with our multidisciplinary team to provide rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.

Key Responsibilities:

• • Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group home settings for adults with intellectual disabilities
  • Utilize current research-based approaches to care to maximize functional independence for clients
  • Work collaboratively with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals in their living environment
  • Educate clients, family, and community caregivers on care plans to achieve a team approach to maximizing functional independence
  • Perform evaluations, treatments, and proper discharge planning for clients using our outpatient EMR platform to deliver and document individualized treatment approaches

Requirements:

• A current NJ Occupational Therapist license
• Prior home care experience preferred, but not required
• A minimum of one year of recent work experience as an Occupational Therapist
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma
• Ability to work independently and manage time effectively
• Strong interpersonal skills, organizational skills, and problem-solving skills
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
