Job Summary

The Occupational Therapist (OT) is responsible for implementing standards of care for occupational therapy services and adhering to all conditions outlined in the Service Agreement. This role requires a strong commitment to providing quality care to patients in their homes.

Responsibilities
  1. Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity.
  2. Evaluate the home environment for hazards or barriers to independent living, identify equipment needs, and participate in the development of the total plan of care.
  3. Develop treatment programs and establish goals for improved function, communicate plans of care to referring physicians and other organization professionals, and teach new skills or retrain patients in daily activities.
  4. Maintain appropriate clinical records, clinical notes, and reports to physicians any changes in patients' conditions, and submit these records in accordance with organization policy.
  5. Maintain contact and communication with other personnel involved in patients' care to promote coordinated and efficient care.
  6. Attend and contribute to in-services, case conferences, and other meetings as required by organization policy to ensure coordinated and comprehensive plans of care for patients.
  7. Identify patient and family/caregiver needs for other home health services and refer as appropriate, instruct patients' family/caregiver and other organization healthcare personnel in patients' treatment regimes when indicated and appropriate, and supervise certified occupational therapy assistants according to organization policy and state regulations.
  8. Participate in discharge planning for patients.
Qualifications
  • Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Licensed to practice as an OT in the State of Florida.
  • Two years of clinical experience as an occupational therapist.
  • Current BLS Certification.
  • Demonstrates good verbal and written communication and organization skills.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
