Kintegra Health seeks a skilled Advanced Practice Provider to join our team. As a valued member of our healthcare staff, you will be responsible for delivering high-quality patient care and working closely with our clinical team to provide exceptional medical services.

About the Role:

  • Engage in a patient-centered approach to providing care for patients and their families.
  • Provide medical evaluation and treatment using standard medical procedures as part of the clinical care team. Respond to emergencies and answer medical questions.
  • Obtain and document appropriate chief complaint, past medical, family, and social history; review of systems, examination, medications, and allergies; assessment and plan for each patient; provide patient education as needed.
  • Prescribe medications, administer treatments, and order diagnostic tests as necessary.
  • Supervise patient care by non-clinic agencies such as specialty providers, home health care, nursing homes, or hospices.
  • Available to patients personally or through clinical staff to answer questions and relay information regarding their care.
  • Educate patients and/or families on the nature of disease, provide instruction on proper care, self-management, and treatment, and work with patients to develop an individualized care plan.

Key Responsibilities:

  1. Develop and implement effective treatment plans tailored to individual patient needs.
  2. Maintain accurate and up-to-date patient records, ensuring timely completion of documentation.
  3. Collaborate with other healthcare professionals to ensure seamless patient care and optimize outcomes.
  4. Participate in quality improvement initiatives, contributing to the development of best practices and policies.
  5. Perform other duties as assigned by supervising physician, Medical Director, Practice Manager, and/or Kintegra Health executive leadership team members.
