Job Title: Clinical Care Specialist

At University Hospitals, we are seeking a highly skilled and compassionate Clinical Care Specialist to join our team. As a key member of our healthcare team, you will provide high-quality patient care in the post-acute setting.

About the Role:
  • This is a collaborative position with a UH physician to provide care for UH patients at King David Center nursing facility.

You will work closely with our interdisciplinary team to deliver exceptional patient care, including skilled and long-term care, as well as possibly some assisted living patients.

Responsibilities:
  1. Initiates and maintains positive relationships with patients/customers (10%)
  2. Initiates and maintains positive relationships with co-workers (10%)
  3. Takes responsibility for self-development and supports a learning environment (10%)
  4. Displays commitment to the mission of the hospital and its values (10%)
  5. Performs patient assessment and assigns diagnosis based on recorded assessment data and current scientific and current research (20%)
  6. Establishes plan of care and implements interventions in accordance with carepaths, protocols, and standards of care and current research (15%)
  7. Collaborates with other health care providers in an interdisciplinary approach to provide care to the patient and family (5%)
  8. Contributes to the educational development of staff, patients, family, and self (5%)
  9. Demonstrates progressive leadership and effective management (5%)
  10. Provides education through role modeling and in-services to meet identified learning needs and to promote professionalism (5%)
  11. Advances own professional development through self-directed learning, continuing education, and participation in peer review (5%)

Requirements:

  • MSN or Master's Degree from an accredited PA educational program
  • 1+ years of experience as a NP or PA
  • Demonstrated leadership skills
  • Demonstrated skills in interpersonal relationships
  • Computer skills to access patient medical data from UHHS mainframe
  • Working knowledge of Microsoft Office and Outlook

Estimated Salary: $80,000 - $120,000 per year
