On Lok PACE is a non-profit organization dedicated to enhancing the quality of life for our participants.
Focus on Elder Care:
The PACE (Program of All-Inclusive Care for the Elderly) model was created to address the numerous challenges associated with caring for vulnerable seniors. This approach emphasizes the importance of providing care within the community to promote the well-being of seniors with chronic care needs and their families.
Participants, along with their families and caregivers, often encounter various challenges, such as arranging transportation for medical appointments, managing medications, coordinating care among multiple healthcare providers, and maintaining social connections. On Lok PACE participants benefit from in-home care services and transportation to a PACE center for primary medical care, social activities, and additional senior care services.

Position Summary: Under the guidance of the Supervisor/Manager, the Caregiver III is responsible for delivering personal care services to On Lok participants receiving home care and/or day health center services.
Key Responsibilities:
- May perform duties of Caregiver I and/or Caregiver II.
- Supervises and coordinates onboarding and ongoing training for other caregivers (I, II, III) utilizing manuals and formal training resources.
- Exhibits leadership by actively participating in team discussions and care planning as necessary.
- Oversees the work of other caregivers; manages more complex cases in collaboration with RN/PT/OT, social workers, and physicians. May serve as a backup to the Caregiver Lead.
- Acts as a mentor to all caregivers.
- Promotes safe practices and encourages the use of appropriate equipment to complete tasks.
- Prepares treatment areas for participants by adhering to established clinical procedures and protocols.
- Executes participant care tasks and accurately documents actions and observations on the designated forms.
- Maintains a flexible and positive attitude while working across various settings to meet participant and organizational needs.
- Actively participates in departmental meetings and family conferences as required.
- Performs other related duties as assigned.
Qualifications (Knowledge, Skills, Abilities):
- HHA/CNA certification required; relevant On Lok experience may be considered in lieu of certification.
- At least one year of experience working with frail or elderly individuals.
- Familiarity with the PACE model gained through personal experience or research.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Current Basic First Aid and CPR certification is required.
- Fluency in English (verbal and written) is essential.
- Able to translate/interpret basic information for participants and other caregivers.
- Basic computer skills (e.g., email communication, calendar management, electronic health record documentation).
- Physical requirements: Ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and perform repetitive motions. Must possess visual acuity and be able to lift/move weights within individual physical capabilities, utilizing appropriate lifting devices or requesting assistance when necessary.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
