Nurse Practitioner Home Visits

We are seeking a dedicated and compassionate Registered Nurse to join our team in Greenwich, CT. As a Registered Nurse with LHC Group, you will have the opportunity to provide high-quality care to patients in their homes, while enjoying a flexible schedule and opportunities for career growth.
Responsibilities- Provide and direct provisions of nursing and personal care to patients in their homes, as prescribed by the physician or as authorized by the state/program.
- Coordinate the plan of care with other health care professionals involved in care and help to achieve and maintain continuity of patient care.
- Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements with other community providers.
- Ensure plan of care frequency and duration meets the needs of the patient and initiates plan of care revisions as needed with physician approval.
- A minimum of one year experience as an RN preferred.
- Must have current RN licensure in state of practice.
- Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.
- Current CPR certification required.
