Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at Millennium Home Health Care, Inc. as a Home Health Care Specialist. As a key member of our team, you will provide high-quality nursing care to patients in their homes, promoting independence, dignity, and comfort.

Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  • Develop and implement individualized care plans to address patients' unique needs and goals.
  • Provide skilled nursing services, including wound care, medication management, and education on disease management and prevention.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Current RN licensure in the state of practice.
  • Basic Life Support (BLS) certification.
  • Experience in home health care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
