Our experienced physical therapists work collaboratively with patients, families, and other healthcare providers to develop and implement tailored treatment plans that promote optimal recovery and function.
About the TeamOur interdisciplinary care team includes nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers – all working together to deliver comprehensive patient care and support. We prioritize open communication, respect, and empathy, creating a collaborative environment where everyone feels valued and supported.
Responsibilities and Expectations- Provide skilled physical therapy services to patients in their homes, including assessments, treatment planning, and implementation.
- Collaborate with multidisciplinary team members to ensure seamless communication and coordinated care.
- Maintain accurate and timely documentation of patient progress, treatment plans, and outcomes.
- Participate in quality improvement initiatives and contribute to professional development opportunities within the organization.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
