Job Summary

We are seeking a highly skilled Social Worker to join our team at The Regional Medical Center of Memphis. As a Social Worker, you will provide social work services and counseling to patients and their families, working closely with physicians and other healthcare team members to ensure comprehensive care.

Key Responsibilities
  • Conduct thorough assessments of patients' social and emotional needs, identifying areas for intervention and developing effective plans to address them.
  • Provide crisis intervention services, counseling, and education to patients and families, promoting coping mechanisms and enhancing overall well-being.
  • Collaborate with healthcare team members to coordinate discharge planning, ensuring a smooth transition for patients and families.
  • Develop and maintain relationships with community service providers, identifying resources and services to support patients' needs.
  • Document all interactions, assessments, and interventions in a timely and accurate manner, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements
  • Bachelor's Degree in Social Work or related field from an accredited college or university.
  • Qualified by education, training, or experience to work with adolescent, adult, and/or geriatric patient populations.
  • Current certification as a Licensed Baccalaureate Social Worker (LBSW) in the State of Tennessee (preferred).
  • Minimum 1 year of social work experience (preferred).
