Long Term Care LPN Position

The Long Term Care LPN is responsible for delivering high-quality patient care in a Long Term Care/Assisted Living Facility. This role requires the ability to make clinical judgments, think critically, and coordinate patient care. The successful candidate will perform according to nursing standards and be accountable for managing patient care and assisting others in the management of patient care.
Key Responsibilities:
Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility
Perform focused interviews to identify specific patient needs
Assess signs and symptoms indicating physiologic and psychological changes in the patient's condition
Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses
Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions
Identify and prioritize actions based on patient care requirements
Monitor consistency between patient plan of care and actions of medical staff
Perform interventions according to identified priorities, plan of care, and facility policies and patient outcome standards
Provide a safe, comfortable, and therapeutic patient environment
Initiate and evaluate patient and/or family education
Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions
Document patient's response to nursing and other interventions
Use clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit, including establishing priorities
Monitor safety and effectiveness of the environment and equipment
Participate in activities to facilitate good interpersonal communications with the chosen facility
Support professional practice by observing the following: Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patient's Bill of Rights; and 6) Amergis Healthcare, nursing and client hospital policies
Requirements:
Current Licensed Practical Nurse License for the state in which the nurse practices
Ability to effectively elicit/provide information to and from appropriate individuals via strong spoken, written, listening, and nonverbal communication skills
Current Health Certificate (if applicable)
Current PPD or Chest X-Ray
One year prior professional nursing experience preferred
Current BLS card
Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater
Successful completion of Background Screening Process
