Job Summary

Key Responsibilities
  • Provide physical therapy services to patients in post acute care settings, including hospitals, rehabilitation centers, and skilled nursing facilities.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Maintain accurate and detailed records of patient progress and treatment plans.
Requirements
  • Graduate of an accredited physical therapy program.
  • Current licensure or registration in the state of Colorado.
  • At least one year of experience in post acute care or a related setting.
  • CPR/BLS certification.
