About the Role:
This is a full-time position offering a competitive salary of $80,000 per year, plus benefits. You will work closely with our interdisciplinary care team to assess patient needs, develop individualized care plans, and coordinate services to ensure a high level of quality care.
Key Responsibilities:
- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Coordinate services to meet patient needs, including medical care, palliative care, and emotional support.
- Work closely with families and caregivers to provide education and support throughout the care process.
- Maintain accurate records and communicate effectively with the healthcare team to ensure seamless care delivery.
Requirements:
- Master's degree in nursing or a related field and current RN licensure in the state of practice.
- 2+ years of experience in hospice care or a related field, with a focus on clinical expertise and interpersonal skills.
- Excellent communication, problem-solving, and leadership skills, with the ability to work effectively in a fast-paced environment.
- A strong understanding of hospice care principles, practices, and regulations, with a commitment to delivering high-quality patient-centered care.
