Hospice RN Case Manager Position

Allyon, Inc. is a leading IT and Healthcare Services firm dedicated to helping talented individuals achieve their career goals while assisting clients in building quality teams. We are seeking a skilled Hospice RN Case Manager to join our team for a 13-week assignment, with the potential to extend.
Key Responsibilities:- Provide high-quality care to patients in their homes, ensuring their comfort and well-being.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Conduct thorough assessments, identify patient needs, and coordinate services to meet those needs.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
- Manage patient records, maintain accurate documentation, and adhere to regulatory requirements.
- NH or Compact RN license.
- At least 1 year of experience in hospice care, preferably in home settings, within the past 3 years.
- Experience with HCHB (Home and Community-Based Services) is highly desirable.
- BLS (Basic Life Support) certification.
