Oncology Nurse Clinician

We are seeking an experienced Oncology Nurse Clinician to join our team at Northwestern Memorial Healthcare. This is a full-time, salaried position at 40 hours per week located at the McHenry clinic. The successful candidate will work in a multidisciplinary environment, coordinating care for patients with cancer and their families.
Key Responsibilities- Assess and reassess patients and families for ongoing care needs, identifying high-risk criteria and initiating referrals to appropriate resources.
- Collect and analyze data, coordinating multidisciplinary care and appointments for new patients.
- Collaborate with patients, families, physicians, and ancillary staff to create a plan of care that indicates collaboration and communication.
- Provide education to patients, families, and caregivers regarding diagnosis, process, therapies, and testing.
- Assist with transitions of care and providers.
- Communicate effectively and work cooperatively with other team members.
- Completion of an accredited Registered Nurse program with an RN License in the state of Illinois.
- Current nursing experience within the past 5 years.
- American Heart Association BLS CPR certification.
- Bachelor of Science in Nursing.
- Oncology Certified Nurse.
