About the Role: We are seeking a skilled and dedicated Home Health Physical Therapy Assistant to join our team in Jacksonville, Florida.
Job Summary: As a Home Health Physical Therapy Assistant, you will play a vital role in providing high-quality patient care and supporting the well-being of patients in their homes.
Responsibilities:
- Under the supervision of a licensed Physical Therapist, perform treatment and provide exercise instruction and patient education aimed at improving/enhancing the patient's well-being while following the established plan of care.
- Instruct and aid patients in evidence-based treatment within the scope of the physical therapy assistant and according to state laws governing physical therapy assistants.
- Observe, record, and report to the supervising PT, nurse supervisor, and/or physician the patient's response to treatment and changes in the patient's condition.
- Cordinate care with other members of the healthcare team as appropriate.
- Participate in staff conferences and committees as requested by the agency.
Requirements:
- Current licensed, registered, or certified to assist in the practice of physical therapy in the state of practice.
- Current CPR certification is required.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Location: This role is based in Jacksonville, Florida, with visits to patients throughout the service area.
