Job Description
The RN Case Manager plays a vital role in our team, overseeing the delivery of home care services and ensuring seamless coordination between healthcare providers, patients, and families. Key responsibilities include conducting client assessments, developing care plans, and coordinating with other disciplines to evaluate client responses to interventions. This position requires strong leadership skills, with the ability to direct and supervise Home Health Aides and LPNs.
Responsibilities
The successful candidate will be responsible for:
- Conducting client assessments, suggesting level of needed service packages to client and their family.
- Developing plans of care with the participation and consent of the client and/or caregiver/family member or legal representative.
- Scheduling, planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems.
- Monitoring side effects, drug interactions, dosages, and implementing changes per physician instructions.
- Documenting significant changes in condition of clients, including incidents, unexplained injuries, medication errors, loss of client property, or evidence of client or family dissatisfaction.
- Analyzing data gathered from assessment and records reviews, and acting promptly to resolve problems.
Requirements
To succeed in this role, you must have:
- Graduate of an accredited school of nursing with RN licensure in the state of Indiana.
- One year of home health care experience preferred.
- One-year supervisory/management experience with direct care staff preferred.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.
Compensation
We offer a competitive salary of $90,000 - $120,000 per year, depending on experience, along with comprehensive benefits and opportunities for professional growth.
