Key Responsibilities
  • Provide high-quality patient care in a home health setting
  • Assess vital signs, including blood pressure, heart rate, respirations, and temperature
  • Develop and implement individualized care plans
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Manage patient records and maintain accurate documentation
Requirements
  • Registered/Licensed with a state board of nursing
  • Valid CPR/First Aid Certification from the American Heart Association
  • Ability to pass a mandated criminal background check
  • Bilingual (English and Spanish) preferred
  • Strong decision-making skills and ability to work independently
Preferred Qualifications
  • Experience with home health or private duty services
  • Knowledge of assessing vital signs and providing care to clients with complex needs
  • Strong communication and organizational skills
  • Ability to work effectively in a fast-paced environment
