Patient Care Coordinator

Optum Primary Care Midvale Clinic is seeking a detail-oriented and patient-focused Associate Patient Care Coordinator to join our team. The ideal candidate will have excellent communication skills and be able to work cooperatively with all members of the care team to support the vision and mission of the organization.

Key Responsibilities:

  • Centralized scheduling, making appointments, and outreach to patients
  • Use call processing and other applicable functionality in the EHR to take messages and communicate with clinicians
  • Greet patients and conduct check-in process
  • Monitor daily reminder call logs to reschedule appointments as needed
  • Close payment batch(es) at the end of each shift, assemble and balance batch(es) according to cash control procedures
  • Maintain knowledge of current OSHA and Optum policies
  • Assist coworkers as needed to assure smooth office operation and delivery of excellent service through teamwork

Requirements:

  • High School Diploma/GED (or higher) OR 2+ years of customer service experience
  • CPR certified or able to obtain certification within 30 days of hire
  • 3+ months of customer service experience
  • Basic level of computer proficiency (including Microsoft Office)

Preferred Qualifications:

  • Experience working with an electronic health record
  • Knowledge of medical terminology
  • Bilingual (English / Spanish)
  • Office Administrative / Receptionist / Clerical experience in a medical environment
  • Excellent communication skills and phone etiquette
  • Ability to multi-task
