We are seeking a skilled Licensed Practical Nurse to join our team as a staff position. As an LPN, you will be responsible for providing direct care to residents and supervising nursing personnel to ensure high-quality care delivery.
Key Responsibilities- Supervise nursing personnel to deliver nursing care and coordinate care delivery to meet resident needs
- Provide direct care to residents, including administering medications and performing treatments
- Conduct regular assessments of resident needs and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery
- Participate in quality improvement initiatives to enhance resident outcomes
- 2-3 years of clinical experience as an LPN/LVN
- Gerontological nursing experience, including dementia population care
- Valid and current LPN/LVN state license
- Ability to read, write, and speak English in an understandable manner
