We are seeking a dedicated and compassionate Full-Time Registered Nurse to join our team at At Home Healthcare. As a valued member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role:

  • As a Full-Time Registered Nurse, you will be responsible for providing high-quality clinical services to our patients in the comfort of their own homes.
  • You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • With a focus on patient-centered care, you will provide education and support to patients and their families, empowering them to take an active role in their healthcare.

Responsibilities:

  • Initiate, develop, implement, and make necessary revisions to the plan of care in collaboration with the physician and other healthcare professionals involved in care.
  • Directly and/or indirectly supervise care provided by home health aides and licensed practical vocational nurses, providing instruction as appropriate, and assigning tasks according to State and federal regulations.
  • Provide high-quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Accurately and timely complete comprehensive assessments (OASIS) including medication reconciliation.
  • Accurately and thoroughly document patient visits per policy, and payer requirements and syncs timely per LHC policy.

Requirements:

  • Current RN licensure in the state of Arizona.
  • Current CPR certification.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle.
