Nursing Role

We seek an experienced Licensed Practical Nurse (LPN) to join our team as a Skilled Nursing Care Specialist. The ideal candidate will have 1-3 years of experience in a long-term care setting, preferably in a supervisory role.

Responsibilities

  • Supervise and lead a team of Certified Nursing Assistants (CNA)/State-Tested Nursing Assistants (STNA) and other team members in providing high-quality patient care.
  • Develop and implement individualized care plans for patients, ensuring comprehensive and timely care delivery.
  • Maintain accurate records of patient care, progress, and treatment outcomes.
  • Collaborate with physicians and interdisciplinary teams to ensure seamless care coordination.
  • Implement effective strategies for preventing complications, stabilizing patient conditions, and promoting overall well-being.

Qualifications

  • Licensed Practical Nurse (LPN) licensure in the state.
  • At least 1-3 years of experience in a long-term care setting, preferably in a supervisory role.
  • CPR certification or acceptable exemption required.
