Job Summary

We are seeking a skilled Care Management Lead to join our team. As a Care Management Lead, you will be responsible for coordinating care services for adult and pediatric members, ensuring they receive comprehensive and integrated support to achieve optimal health outcomes.

Responsibilities:

  • Conduct thorough assessments to identify member needs and develop personalized care plans
  • Collaborate with healthcare providers, social workers, and therapists to ensure seamless care coordination
  • Maintain accurate records of assessments, care plans, and interactions with members
  • Ensure compliance with relevant regulations and standards
  • Communicate effectively with members, families, and healthcare teams to address concerns and update care plans as needed

Requirements:

  • Associate degree in a related field (e.g., healthcare, social work) and minimum 2 years of experience in healthcare or care coordination
  • Current and valid driver's license, proof of auto insurance, and BLS certification from the American Heart Association
  • Demonstrated ability to work independently and collaboratively within a team environment
