Job Title: Permanency Client Service Agent

This is a challenging and rewarding role that requires a strong commitment to supporting families and children in need. As a Permanency Client Service Agent, you will work closely with clients to provide support services, including arranging appointments and transportation, monitoring and supervising visits, and assisting with daily living activities.

Key Responsibilities:
  • Transport clients to appointments, visits, and meetings, and monitor and supervise visits to ensure their safety and well-being.
  • Document activities and information in FACTS as required by agency policy and procedures.
  • Provide support to case workers, including entering payments into FACTS and assisting with client-related projects.
  • Identify potential community-based services and share information to assist staff in promoting safety, permanency, and well-being.
Requirements:
  • High School Diploma or Equivalency.
  • Valid Driver's License.
  • Ability to work under stress and frequent time constraints.
Working Conditions:

This role requires working in an office setting, with potential for late hours and weekends. You will be exposed to regular periods of video display terminal and keyboard usage, as well as stressful situations. Some statewide travel may be required.
