Job Title: Care Coordinator for Geriatric PopulationAbout the Role

The Care Coordinator will play a vital role in ensuring the well-being and comfort of our geriatric clients. This is a full-time position, Monday through Friday, 8am to 4pm, with no nights, weekends, or holidays.

Location: Beverly, MA

Responsibilities:

  • Collaborate with interdisciplinary teams to conduct initial assessments, develop care plans, and perform ongoing re-assessments of participant care.
  • Attend IDT meetings, actively participate by sharing pertinent information, providing follow-up on assigned tasks, and contribute to developing participant care plans.
  • Complete all assigned assessments, including initial, semi-annual, annual, service requests, and significant events.
  • Evaluate participants' psychosocial needs and provide supportive counseling, working closely with behavioral health providers.
  • Facilitate hospital, rehabilitation, and nursing home admissions and discharges as determined by the Interdisciplinary Team, ensuring PASRR documentation is completed for NH admission.
  • Assist in converting participants from community to long-term care, collaborating with Medicaid Specialists, skilled nursing facilities, and caregivers to complete the process.
  • Arrange and facilitate family meetings as needed.
  • Refer participants and families to relevant community services and act as liaison/advocate with community organizations on behalf of participants.
  • Maintain accurate and timely social service documentation in participants' medical records.
  • Conduct participant council meetings as assigned.
  • Work collaboratively with the Director of Social Work and Behavioral Health provider to ensure guardianship is up-to-date, educate participants about healthcare proxies, and assist with completing HCP forms.
  • Collaborate with the fiscal department to maintain participant insurance benefits and document required fiscal information in medical records.
  • Review care plans with participants, guardians, and/or activated healthcare proxies as assigned.
  • Authorize home care and other approved services in a timely and accurate manner.
  • Perform home and/or skilled nursing facility visits to assess participants as necessary.
  • Collaborate with the Palliative Care team, assisting with end-of-life planning as indicated.
  • Provide timely communication to relevant staff regarding disenrollment, conversion to long-term care, transfer of sites, participant/caregiver demographic changes, and allegations of abuse.
  • Report allegations of abuse to the appropriate state agency, offer support and resources to participants, and complete required documentation.
  • Ability to pass a fit test; the position requires wearing a mask where a seal is critical, and the incumbent must not have facial hair that interferes with a tight respirator seal.
  • Perform other duties as required.
  • Frequent local travel.
Qualifications

To be considered for this role, applicants must possess:

  • A current Master's-level Social Work license (L.I.C.S.W. or L.C.S.W.) in the Commonwealth of Massachusetts.
  • A minimum of one year of experience in Social Work, providing traditional clinical or case management services with a geriatric population.
  • Current C.P.R. Certification or ability to become certified.
  • Strong verbal, written, and listening skills, with the ability to multitask in a fast-paced environment.
