We are proud to be designated as an Accredited Chest Pain Center, an Accredited Heart Failure Center, and a Certified Primary Stroke Center.
Key Responsibilities:
- Maintain an active Florida RN License.
- Possess a minimum of 2 years of acute care experience, with 1-2 years in Case Management.
- Preferred experience in Case Management within an acute care setting.
- Demonstrate knowledge in discharge planning.
- Facilitate a collaborative approach to assessment, planning, and care coordination, ensuring comprehensive health needs are met.
