Physical Therapist

Job Summary:
We are seeking a skilled Physical Therapist to join our team at Ellis Bragg LLC, a leading provider of post-acute care. As a Physical Therapist, you will play a critical role in helping our patients achieve optimal health and wellness.
Key Responsibilities:
- Develop and implement effective treatment plans for patients with various physical therapy needs.
- Treat patients according to physician-prescribed plans, ensuring the highest level of care and attention to detail.
- Collaborate with the nursing department to provide Restorative Aide training and support.
- Supervise Physical Therapy Assistants in patient care, adhering to state regulations and ensuring the highest standards of care.
- Communicate patient progress and plans with the healthcare team, ensuring seamless coordination of care.
- Attend Patient Care Conferences and Rehabilitation Conferences as needed, staying up-to-date on the latest best practices and research.
- Participate in staff in-services training, sharing knowledge and expertise with colleagues.
- Document evaluations, progress notes, and recertification in compliance with PT Board regulations, maintaining accurate and detailed records.
- Record treatment changes following policies, ensuring consistency and continuity of care.
- Provide family/caregiver training for discharge, empowering patients and their loved ones with the knowledge and skills needed for successful recovery.
- Assist in discharge planning, ensuring a smooth transition from our facility to the patient's home or next level of care.
- Recommend and facilitate ordering of durable medical equipment for patient mobility, enhancing the patient's quality of life and independence.
- Report equipment issues promptly, ensuring timely resolution and minimizing disruptions to patient care.
- Foster positive relationships with patients, families, and colleagues, promoting a culture of respect, empathy, and compassion.
Qualifications:
- Valid Physical Therapist License
