Job Title: RN Care Delivery Field Supervisor

CenterLight Health System is seeking a highly skilled and experienced RN Care Delivery Field Supervisor to join our team. As a key member of our management team, you will be responsible for overseeing clinical services provided at participants' homes, ensuring high-quality care and compliance with care plans.

Key Responsibilities:
  • Delegate responsibility to assigned personnel and supervise in collaboration with the Care Delivery Manager to ensure quality care and compliance with care plans.
  • Assess and evaluate participants' health care needs, considering physical, mental, and environmental factors.
  • Supervise, instruct, and guide Community Health Nurses (CHNs) and Home Health Aides (HHAs) in delivering quality home health care services.
  • Conduct joint visits with CHNs to provide supervision and competencies.
  • Collaborate with the Interdisciplinary Team (IDT) to develop comprehensive care plans based on member-specific needs and physician orders.
  • Complete, maintain, and submit required documentation in a timely manner, adhering to CLHC standards and federal/state regulations.
Requirements:
  • Current active and unrestricted license and registration in New York State as a Registered Nurse.
  • Minimum of one year of experience in home care as a community health nurse in a Certified Home Health Agency, Long Term Home Health Care Program, and/or Licensed Agency.
  • Excellent organizational and presentation skills.
  • Computer/laptop proficiency required.
  • Managed long-term care insurance experience beneficial.
  • Vehicle and current New York State driver's license preferred.
Physical Requirements:
  • Standing for up to 6 hours a day.
  • Sitting/Stationary positions for up to 6-8 hours a day.
  • Lifting/Push/Pull up to 50 pounds of equipment, baggage, supplies, and patients safely.
  • Bending/Squatting to care for patients and use medical supplies.
  • Stairs/Steps/Walking/Climbing to access work areas.
