We are currently seeking a Board Eligible/Board Certified Family Medicine Physician for a full-time role focused on transitional care. This position is based in an outpatient clinic setting, primarily aimed at providing coordinated care to facilitate patient transitions to standard primary care.
The selected candidate will engage in new consultations stemming from referrals from emergency and inpatient services. Ideal candidates should possess Board Eligibility/Certification in Primary Care and will collaborate closely with a team of health advocates and various subspecialists.
Key Responsibilities:- Become part of a reputable team of primary care physicians.
