Key Responsibilities- Deliver high-quality nursing care, including proficiency with direct skills such as head-to-toe assessments, wound care, IV, catheter management, disease process education, medication management, etc.
- Perform necessary documentation in the Electronic Medical Record (EMR) according to agency policy, including consent for treatments, admissions, evaluations, assessments, plan of care development, prior level of function, discharge planning, and ongoing physician orders.
- Collaborate effectively with the case manager by delivering the right visit/intervention at the right time.
- Demonstrate ability to critically think, plan, organize, and problem-solve around the ongoing needs of the client.
- STAR mentality (Selfless, Tough, Accountable, Respectful)
- At least one year of experience as a nurse in a home care setting within the last 5 years preferred
- Must be licensed in the state of Nebraska or compact-state as an RN
- Graduate from an accredited nursing program
