Job Summary

The Social Services Director at Life Care Center of America is responsible for planning, organizing, and directing the overall operation of the Social Services department. This role ensures that the medically related emotional and social needs of patients are met in accordance with all applicable laws, regulations, and Life Care standards.

Key Responsibilities

  • Develop and implement a social services program that meets the needs of patients and adheres to State, Federal, corporate, and division guidelines.
  • Act as a patient advocate and provide education to staff on patient rights.
  • Chart patient information accurately and in a timely manner.
  • Assist patients and families with education, financial planning, and liaison with community agencies.
  • Recruit, select, hire, train, evaluate, counsel, and supervise social services staff.
  • Exhibit excellent customer service and a positive attitude towards patients.
  • Assist in the evacuation of patients during emergencies.
  • Demonstrate dependable, regular attendance and use reasoning skills and good judgment.
  • Communicate effectively on an interdisciplinary team and read, write, speak, and understand the English language.

Requirements

  • Currently registered/licensed in applicable State and maintain an active license in good standing throughout employment.
  • Bachelor's degree if working in a facility with 120 or more beds (see State law).
  • Two (2) years' experience in health care social work preferred.
