Job Title: Case Manager RN, Home Health

Job Summary:

An experienced Registered Nurse (RN) is sought to manage patients' care in a home health setting. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently as part of an interdisciplinary team.

Responsibilities:

  • Plan, organize, and direct patient care services to ensure optimal quality and adherence to regulations.
  • Coordinate with the interdisciplinary team to implement patient care plans and manage visit utilization and expenses.
  • Assist in agency-wide orientation, competency evaluations, and consultation with clinical staff.
  • Provide direct care to patients, including assessment, planning, and implementation of care plans.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions.

Requirements:

  • Graduate of an accredited School of Nursing.
  • Current RN licensure in California.
  • Basic Life Support (BLS) certification.
  • Valid driver's license and automobile insurance.
  • 2 years of recent relevant experience.

Preferred Qualifications:

  • Knowledge of patho-physiology and accepted treatment protocols for common home care diagnoses.
  • Basic safety and infection control principles.
  • Clinical assessment skills.
  • Ability to recognize special needs of Hospice patients and others in the home.

Work Environment:

This position may regularly work with hazardous or potentially hazardous drugs, as identified by the National Institute for Occupational Safety and Health (NIOSH), for purposes of USP 800.

Pay Range:

$65.17 to $86.66 per hour.

The salary range for this role may vary above or below the posted range as determined by location. Compensation takes into account several factors, including a candidate's experience, education, skills, licensure, and certifications, department equity, training, and organizational needs.
